Please note: We have a new Method of Payment/Authorization Form that must accompany each order. Only one authorization form is needed per show for multiple services and materials, regardless of method of payment. The form is already attached as page 2 on each of the order forms below; if you need a separate copy, you may click on the link above or the image below left to get a single form.
